When you supply your personal details to CLR Physiotherapy they are stored and processed for the following reasons.

•  We need to collect personal information regarding your health in order to provide you with the best possible treatment and advice.
• We need to be able to contact you regarding appointments and any enquiries that you may have.

Please note that all data collected will be used and held in accordance with the Data Protection act and General Data Protection Regulation (GDPR).

We are regulated by Health Care Professions Council and registered with the Information Commissioner’s Office.

We have a legal obligation to retain your records in accordance with our professional guidelines.

Your records are stored on paper notes in locked filing cabinets or on our computer which is password protected.

We will never share your information with anyone without your written consent.

You have the right to see what personal data of yours we hold in accordance with our obligations under Data Protection laws.
